Common medication list
Background
Surveys from Norway and Sweden indicate that as many as 5-10 percent of all admissions to hospital can be related to errors in medication. Numbers from the Norwegian Board of Health show that these errors lead to a minimum of 10-15 deaths and 50-100 injuries a year. Most likely the dark figure is high.
The sources of errors can be divided into errors in prescribing, wrong doses and errors in administration of the medication. Communication between the different health professionals and institutions seems to be a crucial point.
Today the routines for communication in some cases are insufficient or do not function as intended. We presume that a computer-based tool for information exchange and an adjustment of routines would minimize the problem. This is the background for the pre-project NST is undertaking this spring.
Project description
The project's main goal is to write several applications for funds to conduct a main project for developing the actual service.
